Family Medical Leave Act (FMLA)
Information
MATERNITY LEAVES
Please give as
much advanced notice as possible
to the Personnel Department
1. Fill out a maternity leave notification
form. Attach a doctor's note giving date(s) regarding your ability to
work/not work. These dates can be changed if necessary.
2. Fill out Sick Bank
papers and send them to Dorothy Yope (Lobbestael). E-mail her if you
have any questions.
3. A copy of your doctor's
note must accompany the sick bank form with the original note forwarded to
the Personnel Department (attention Rosemary).
4. Six (6) week maternity
leave starts the day the baby is born. Eight (8) weeks for a C-section.
(Holidays, i.e. Christmas, Easter, mid-winter break, etc. are considered as
part of the 6 or 8 week leave). Your own personal sick time may not be used
to extend a maternity leave
beyond the disability period.
5. Family medical leave
(12 weeks) shall commence at the teacher’s
request following notification to the district of the end of the disability
period (if applicable).
6. A doctor's note is
required stating the exact date of return. Notes may be faxed directly to
the Personnel Office (783-6311). A member can apply for additional time
(unpaid leave) by letter to Mrs. Standel, Asst. Superintendent for
Personnel.
7. Not following the
procedures can result in BIG TIME problems with
the payroll department and your paycheck.
8. Doing everything in a
timely manner assists everyone (Personnel Dept., Payroll Dept. and
Substitute Teacher Caller) in making your leave and your return run
smoothly.
SICK LEAVE
1. Sick days may
be used by bargaining unit members for illness disability, which
shall include all disability caused by or contributed to by
pregnancy miscarriage, childbirth, or related medical conditions;
medical treatment of the member of the bargaining unit or serious
illness of a member of the immediate family of the bargaining unit
member. (Immediate family shall be interpreted as: mother, father,
wife, husband, child, sister, brother, or the family unit living in
the teacher's household).
2. An absence of
seven (7) consecutive days requires a doctor's note. (district
policy)
3. Sick days
cannot be shared between teachers or given to other teachers.
4. Sick Bank days
may not be used for anyone other than a L'Anse Creuse teacher. They
cannot be used for a family member's illnesses.
5. Fill out Sick
Bank papers and return these papers to Beth Broder at Special
Services.
6. A copy of the
doctor's note must accompany sick bank form with the original
doctor's note going to Personnel.
7. Doctor's notes
can be faxed. (Personnel Office, 783-6311)
8. Notify the sub
calling system when you are leaving and when you are returning.
9. Not following

10. Doing
